Our Conwy/Denbighshire Advocacy Service is accessible to adults aged 18 and over and their carers who are ordinarily resident within the county of Denbighshire or Conwy. Eligibility also requires that they are in receipt of, or eligible for, social care support and their situation is described in one or more of the following:

• An individual whose disability/vulnerability affects their ability to self-advocate.
  
  
• An individual aged 18 or over, who is unable to take care of their self or protect themselves from harm or abuse.
  
  
• An individual aged 18 or over who is subject to physical intervention in the management of their care. Carers who are providing care to someone, aged 18 or over, who may need help and support because of their age and vulnerability, physical or mental illness, addiction or disability.
